To make your visit to Cornerstone Christian Counseling more efficient, we have made available for download the forms that you will need to complete before your first session. You may download these forms, complete them in your own time, and bring them with you on your first visit to our office.

You should also contact your insurance company by calling the number on your insurance card prior to coming to your first appointment. Check on your mental health coverage so that you understand the deductible and copay rates. You will also need to check if you require pre-authorization and whether the therapist you have chosen is covered in network or out of network. The website of your insurance carrier may be an additional resource.
